DESCRIPTION & OBJECTIVES
Neuro Continuity of Care is an advanced case management conference featuring nationally recognized experts in the rehabilitation of brain injured patients. This one-day program will give you up to date information about current treatment techniques and available resources that affect persons with catastrophic brain injuries and illnesses. By the end of the conference, participants will be able to:

• Become familiar with the various levels of medical
and rehabilitation care for patients with brain
injuries.
• Learn the various treatment options in rehabilitation
of brain injury patients.
• Become aware of the frequent medical
complications after brain injury.
• Understand the complete definition of Skilled
Nursing Facility (SNF) along with Medicare transfer
requirements including denials and exclusions.
• Understand Medicare benefit days / periods and
payment to SNF based on RUGS system.
• Understand Medicare funded SNF services and
additional SNF services enhancing patient
experience
• Understand the importance of on-site evaluations
versus a fax referral.
• Understand the role and purpose of a community
integration program.
• Understand services provided in Memorial
Hermann TIRR Challenge Program
• Access the clinical appropriateness for discharge to
Home Healthcare versus Home Care.
• Become aware of the funding resources and
discharge issues of Home Health and Home Care.

CONTINUING EDUCATION HOURS

Nurses*, Case Managers, and Social Workers will receive 7.0 contact hours for attending this conference in its entirety. No partial credit will be given. (*H.I.S. Cornerstone is an approved provider of continuing nursing education by the Texas Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.)
